Elastic Stocking

Hand Washing
Every still will begin with address client by name, Introduces yourself to client by name explains procedure, speaking clearly, slowly, and directly, maintaining face-to-face contact, provide privacy and wash your hands.
Lather hands for 20 seconds.
Start on 12 stop at 4
Start on 3 stop at 7
Start on 6 stop at 10
Start on 9 stop at 1
Every skill will end signaling device (call light) is within reach and bed is in low position. After completing skill, wash hands


Denture Care
Line bottom of sink
Rinse denture in moderate temperature running water before brushing then applies toothpaste to toothbrush rinses denture under water, remove gloves and wash hands.
Elastic Stocking
Finishes procedures with no twists or winkles. Heel lined up with heel.

Position On Side
Before turning lowers head of bed. Raises side rail on side to which body will be turned. Don't forget to adjust head pillow.

Foot Care

ROM Of Knee & Ankle
No lotion between toes.
Support knee & ankle 3 cycles of each flexion, extension, dorsi & plantar. Are you experiencing any pain?

Feeds Client Who Cannot Feed self

Modified Bed Bath
Before feeding sit upright 75-90 degrees. Place tray where client can see. Ask client to state name. Candidate sits in chair facing client during feeding. Wipe hands before feeding,. Tell clients what food & beverage are on tray. At the end of meal, candidate cleans client's mouth & hands. Removes food tray. Leave client sitting up for 30 minutes.
No soap on face. You can use the same washcloth for arm. One arm wash under armpit, side of arm & underneath arm. Clean hand & fingernails. Soap, rinse pat dry.

Catheter Care
Holding catheter without tugging, cleans at least 4 inches. Hold at meatus. Moving only in one direction. Use different area of washcloth.

Mouth Care
Ask client to stick tongue out. Sitting upright position 75-90 degrees.

Respirations
Measurement skill
Normal range 12 - 20
Count 1 minute
wash hands & then record.

Peri-Care
Introduce yourself, provide privacy, skill is peri care and wash your hands. Use 4 wash cloths 2 for the front and 2 for the buttocks. Use 3 different areas of the wash cloth soap, rinse & pat dry. End skill, bed low/lock, rinse pat dry basin, remove gloves, call light within reach and wash hands.

Weight
Zero scale out. Put on shoes. Measurement skill wash hands and then record results.


Donning PPE
Puts on gown 1st secure at neck & waist then put gloves on. Tuck sleeves of gown in gloves. Remove gloves 1st & then gown. Watch glove technique.

Add 200 + 22 = 222 pounds

Count for 1 minute
Wash hands before recording.

Measurement Skill before recording, wash hands. After obtaining reading using BP cuff, records both systolic/diastolic pressures exactly
as displayed on the digital screen.

Assists To Ambulate Using Transfer Belt

Check all 4 bed wheels are locked. Put client non-skid shoes on. Count including prearranged signal 1-2-3 I'm sit you up, 1-2-3 I'm stand you up, 1-2-3 feel for the wheelchair, 1-2-3 sit down. Position wheelchair at the head of bed or the end of bed.
Transfer from bed to wheelchair transfer belt

Dresses client with affected (weak) right arm
Face-face. Ask which shirt he/she would like to wear? When undressing start with the good arm first. When dressing start with the affected right arm first.
Check all 4 bed wheels are locked. Put client non-skid shoes/footwear on. Transfer belt buckle in front. Count including prearranged signal like1-2-3 I'm sit you up, 1-2-3 I'm stand you up,1-2-3 sit you down.

Measurement Skill read at eye level. Wash hands and then record.
